Thanks to those who responded to my question. The solution John 
James from Mathsoft provided is:

Hi, the easiest solution is to quit S+, delete 
the _prefs directory and
restart S+. Accept the defaults and this will 
return S+ to its initial
state.

If you have customized menus, as above but move 
_prefs to_prefsKeep. You
should then be able to introduce each of the 
menus from the old in turn.
Given what you've done, I'd recommend the first 
solution as there may be
corruption. If you find that some problems still 
exist, stop S+, delete the
__sum.txt, __sum4.txt, __.sum4i.txt files and 
__nonfi files & restart.

The same is true for the _data directory 
(obviously don't delete it!)
