Through various members of the S-news group, I've become aware of 2
resources in which other folks might be interested.
1) There is some shareware available for estimating mixed-effects logistic
regression for both nominal and ordinal data, and mixed effects Poisson
regression (MIXNO, MIXOR, and MIXPREG). It written by Donald Hedeker in
FORTRAN and has an optional Windows interface. The programs can be
downloaded, along with documentation at:

2) Harvey Goldstein's book, Multilevel Statistical Models, 2nd Edition
(Kendall's Library of Statistics 3) can be downloaded for free at
www.arnoldpublishers.com/support/goldstein.htm
