UNIX S-PLUS world map, Lmake, Gmake and map.builder

Installing the world map (available at http://lib.stat.cmu.edu/maps/)
requires Lmake and Gmake, which aren't included. I just ran into this
problem and I see a few others have also been confused by it.

The code for Lmake and Gmake is obtainable from
http://lib.stat.cmu.edu/general/map.builder .
